Case Middle School’s School-Based Health Center
The North Country Family Health Center, or NCFHS, has been providing medical services to students and the community at Case Middle School for the last 25 years. It started in December 1993 as the North Country Children’s Clinic. In 1996, the clinic opened its second School-Based Health Center, at Watertown High School. Case Middle School is the third school in the Watertown City School District to open a School-Based Health Center. The clinic received a $25,000 grant from the Northern New York Community Foundation to help establish the Center at Case Middle School.
School-based health centers are located in the school and are staffed with a team of health care professionals. This team may include a doctor, nurse practitioner, physician assistant, social worker, and a medical assistant. The school-based health center staff can help children with minor illnesses and injuries and can also be a child’s primary care physician.
North Country Family Health Center
North Country Family Health Center is a Federally Qualified Health Center that provides quality, affordable health care to over 12,000 residents of the North Country each year. Through school-based and community-based locations, NCFHC provides health care to people in need and a range of health-related programs. In addition, the center provides nutritious food vouchers and other support to more than 5,000 pregnant women every month through the WIC program, and helps 4,000 individuals enroll in insurance each year. Other services include special initiatives for high-risk pregnant women and people with unstable housing.
The North Country Family Health Center is a federally qualified health center serving children and adults throughout Lewis and Jefferson counties in Upstate New York. The Center offers six school-based health centers and offers telepsychiatry and operates a four-county WIC program.
Montefiore’s Patient Centered Medical Home
Implementing a patient-centered medical home can have many benefits for a healthcare organization. It can reduce costs, improve care and physician morale. The Montefiore Medical Group in the Bronx, N.Y., has been a pioneer in developing the PCMH model. It recently received NCQA recognition as a patient-centered medical home. Montefiore has also collaborated with the National Center for Quality Assurance (NCQA) to develop PCMH standards for SBHCs.
The SBHCs at Montefiore offer primary care and preventive care to students. The centers also provide dental and mental health care. They have reduced emergency room visits for children and increased school attendance. They also focus on five priority areas of health, such as asthma management, nutrition, physical activity, reproductive health, and emotional well-being.
The SBHCs work with electronic health records to coordinate services. They have access to students’ medical records, which allow them to tailor services to individual needs.
